Coggins Test It is not necessary to fill out the Coggins Test form if you have already completed the Health Certificate form. A Coggins test screens the horse’s blood for antibodies against the Equine Infectious Anemia virus. Some horse owners know this disease as “Swamp Fever,” since it is often transmitted by biting insects such as flies and mosquitoes. The Coggins test is valid for 6 – 12 months depending on your state of destination. Current pictures or an accurate drawing of all markings and brands on the horse is required. If you need a Coggins test for your horse, please call our office 7-10 days prior to departure to set up an appointment for the doctor to take a blood sample. Please fill out all required fields in the Coggins form.Owner's InformationOwner's Name*Your Email*Address*City*State*Zip Code*Horse InformationHorse Name*Registered NameColor*Breed*Age*Date of Birth MM slash DD slash YYYY Sex*ColtFillyGeldingJackJennyJohnMareSpayed MareStallionLeft Side ViewMax. file size: 256 MB.Right Side ViewMax. file size: 256 MB.Head ViewMax. file size: 256 MB.Additional Horse InfoHorse Name 2Registered Name 2Color 2Breed 2Age 2Date of Birth 2Left Side View 2Max. file size: 256 MB.Right Side View 2Max. file size: 256 MB.Head View 2Max. file size: 256 MB.Stable InformationSame as Owner Yes Stable Name*Stable Address*Stable City*Stable Country*Stable State*Stable Zip Code*Additional QuestionsReason for Testing*Change of OwnershipEquine Event PermitHospital/ClinicMovingOtherPleasureRacingReproductive Breeding FacilityRodeoSaleShow/ExhibitionTrainingTransshipmentVeterinary SchoolHave you scheduled an appointment with one of our Veterinarians yet?* Yes No
